What to Expect During Your Visit
A comprehensive geriatric assessment is performed, analyzing medical, functional, mental, nutritional, and social aspects. An individualized care plan is created, which may include adjusted medication, physiotherapy, social support, or referrals to other units.
Specialized Services
Person-centered multidisciplinary approach. Coordination with social work, rehabilitation, nursing, and family. Fall and functional decline prevention plans. Medication adjustment for polymedicated patients. Care in outpatient clinics and hospitalization.
